Group claim · Provider participation

Join the class action and file your own account.

Participation is done in your own name, from your own records. You create an account, sign the join form, then write up each experience with dates and amounts. Every entry stays private to you and the reviewers handling the claim.

  1. 01

    Create your account

Use the sign-in page to register with your email and a password of at least eight characters. An account keeps your submission tied to you alone — nobody else can read what you file.

    Sign the join form

On your member page, confirm your name, contact email, state, the role you worked in, the period involved, and the amount at issue. Tick both consent boxes: one to be contacted about the claim, one to be counted in the group.

    Write up each experience

Add one entry per issue — unpaid or short-paid work orders, fee or deduction disputes, sudden deactivation, scoring problems, classification, or dispute handling. Give dates, amounts, and what you were told, in your own words.

    Attach your documents

Use the source record to log links and upload PDFs or screenshots of work orders, payment statements, messages, and deactivation notices. Each item gets an exhibit reference and a read-only audit entry.

File with your state attorney general

Work the state checklist, then generate a dated complaint draft you can send to your own attorney general. Your filing strengthens the record the group claim relies on.
